Community Support

The mission of Community Support Services is to provide out of clinic comprehensive behavioral services to adults living with severe and persistent mental illnesses who are most in need of ongoing support so they can live as independently as possible in the community. Services for adults are based on a recovery model and assist the individual in setting goals and utilize their strengths to increase independence and quality of life. Interventions assist individuals with the management of symptoms, accessing community resources and in developing relapse prevention strategies and plans. These services include:

Community Support:

Assisting adults with Mental Health Diagnosis and/or Substance Related Disorder through:

  • Engaging the individual in a recovery-based partnership and provides identification of needs, support, HOPE, and encouragement
  • Coordination of services that reduce barriers to accessing services and resources
  • Referring and linking to services and resources
  • Monitoring and Follow-up to determine if services are adequately and effectively addressing the individual's needs

Psychosocial Rehabilitation-Individual:

Providing skills building with adult with Mental Health Diagnosis and/or Substance-Related Disorder through:

  • Providing skills support in the person's self-articulation of personal goals and objectives
  • Assisting the individuals in the development of skills to self-manage or prevent crisis situations
  • Individualized interventions in living, learning, working, other social environments

Supported Employment:

Assisting adults with Mental Health Diagnosis and/or Substance-Related Disorder who express a desire for competitive employment. Services include supports to access benefits counseling: identify vocational skills and interests; and develop and implement a job search plan to obtain employment based on the individual's strengths, preferences, abilities and need.

Community Support:

Providing services to youth with Behavioral Health condition by assisting a youth/family in gaining access to necessary services and creating environments that promote resiliency and support the emotional and functional growth and development of the youth through:

  • Assistance to the youth and family/responsible caregivers in the facilitation and coordination of the Individual Resiliency Plan
  • Planning in a proactive manner to assist the youth/family in managing or preventing crisis situations
  • Individualized interventions
